Bone Densitometry

Steps Involved in IVF:

Procedure Description

Bone densitometry, also known as Dual-Energy X-ray Absorptiometry (DEXA) or Bone Mineral Density (BMD) scanning, is a specialized medical imaging technique used to measure the density of minerals (mainly calcium) in bones. This procedure is most commonly performed to diagnose osteoporosis or assess the risk of fractures by evaluating bone strength. It is a non-invasive, painless procedure, making it a preferred method for many patients who require routine monitoring of bone health.

The process involves using low-dose X-rays to capture images of the bones, typically in the spine, hips, and sometimes the forearm. These are the most vulnerable areas for osteoporosis-related fractures. The machine emits two X-ray beams, with different energy levels, that pass through the bones. By measuring how much of the X-rays are absorbed by the bones, the machine can calculate the bone density, which is compared to a reference value based on age, gender, and ethnic group norms.

Bone densitometry is widely used not only for the diagnosis of osteoporosis but also for tracking the effectiveness of treatment for bone-thinning disorders. The procedure is recommended for postmenopausal women, men over 50, and individuals who have specific risk factors, such as a family history of osteoporosis, long-term steroid use, or lifestyle factors like smoking and excessive alcohol consumption.

Procedure Duration

Bone densitometry is a quick and straightforward procedure. The actual scan time typically lasts between 10 to 30 minutes, depending on the area of the body being examined and the equipment used. Most patients experience no discomfort during the procedure, and since it is non-invasive, it does not require any recovery time, meaning that patients can resume normal activities immediately after the scan.

The procedure begins with the patient lying on a padded table, while the DEXA scanner arm passes over the body, taking measurements. The patient must remain still to ensure accurate readings. Sometimes, cushions or supports are used to position the body correctly, especially when scanning the spine or hips.

For more comprehensive screenings, a full-body scan may be requested, which could take slightly longer. Following the procedure, results are typically analyzed by a radiologist or a specialized technician and sent to the referring doctor. These results are usually available within a few days, depending on the healthcare facility. The doctor will then discuss the results, including whether any follow-up tests or treatments are necessary, particularly if signs of bone thinning or osteoporosis are detected.

Benefits

  • Non-invasive and painless: No surgical instruments are used, and there is no discomfort during the scan.
  • Quick procedure: The scan typically takes between 10 and 30 minutes, with no recovery time needed.
  • Early detection of osteoporosis: Bone densitometry helps identify bone thinning before fractures occur.
  • Monitor treatment efficacy: The scan allows doctors to track the effectiveness of osteoporosis treatments over time.
  • Low radiation exposure: The amount of radiation used in bone densitometry is significantly lower than that of a standard chest X-ray.

With some of the best medical infrastructure in Europe, Spain is a top destination for health-conscious travelers.

Risks & Considerations

  • Radiation exposure: Although the amount of radiation is low, patients with concerns about radiation exposure should discuss alternatives with their doctor.
  • Limited scope of diagnosis: Bone densitometry measures bone density but does not assess the cause of bone loss or other underlying conditions.
  • False negatives: In some cases, early-stage bone loss may not be detected, requiring follow-up tests for more accurate diagnosis.
  • Not suitable for all patients: Pregnant women or individuals with recent fractures may not be ideal candidates for this scan.
  • Reliability based on machine type: Some older machines may provide less accurate results than newer models, so choosing a facility with updated technology is essential.

How to Choose the Right Doctor and Hospital

When selecting a doctor and hospital for bone densitometry, patients should prioritize facilities with accredited diagnostic imaging departments that use state-of-the-art equipment. It is also essential to choose a healthcare provider with a strong reputation for radiology and diagnostic services. Patients can verify whether the clinic uses up-to-date DEXA scanners and offers the latest imaging techniques, as modern technology ensures more accurate results.

Additionally, it is advisable to consult with a healthcare provider who specializes in bone health or osteoporosis management. A well-qualified doctor will not only interpret the bone densitometry results effectively but will also be able to recommend follow-up care or treatment based on the findings. Ensuring that the clinic or hospital is familiar with treating international patients can also help facilitate a smoother experience for medical tourists.

Frequently Asked Questions

What actually happens during hyperstimulation of the ovaries?

The patient will take injectable FSH (follicle stimulating hormone) for eight to eleven days, depending on how long the follicles take to mature. This hormone is produced naturally in a woman’s body causing one egg to develop per cycle. Taking the injectable FSH causes several follicles to develop at once, at approximately the same rate. The development is monitored with vaginal ultrasounds and following the patient’s levels of estradiol and progesterone. FSH brand names include Repronex, Follistim, Menopur, Gonal-F and Bravelle. The patient injects herself daily.

What happens during egg retrieval?

When the follicles have developed enough to be harvested, the patient attends an appointment  where she is anesthetized and prepared for the procedure. Next, the doctor uses an ultrasound probe to guide a needle through the vaginal wall and into the follicle of the ovary. The thin needle draws the follicle fluid, which is then examined by an embryologist to find the eggs. The whole process takes about 20 minutes.

What happens to the eggs?

In the next step, the harvested eggs are then fertilized. If the sperm from the potential father, or in some cases, anonymous donor, has normal functionality, the eggs and sperm are placed together in a dish with a nutrient fluid, then incubated overnight to fertilize normally. If the sperm functionality is suboptimal, an embryologist uses Intracytoplasmic Sperm Injection to inject a single sperm into a single egg with an extremely precise glass needle.  Once fertilization is complete, the embryos are assessed and prepared to be transferred to the patient’s uterus.

How are the embryos transferred back to the uterus?

The doctor and the patient will discuss the number of embryos to be transferred. The number of successfully fertilized eggs usually determines the number of eggs to be placed in the uterus. Embryos are transferred to the uterus with transabdominal ultrasound guidance. This process does not require anesthesia, but it can cause minor cervical or uterine discomfort. Following transfer, the patient is advised to take at least one days bed rest and two or three additional days of rest, then 10 to 12 days later, two pregnancy tests are scheduled to confirm success. Once two positive tests are completed, an obstetrical ultrasound is ordered to show the sac, fetal pole, yolk sac and fetal heart rate.

Embryoscope©

Built into this technology there is a microscope with a powerful camera that allows the uninterrupted monitoring of the embryo during its first hours of life. In this way, we can keep a close eye on the embryo, from the moment when the oocyte is inseminated and begins to divide into smaller and smaller cells, until it can be transferred to the uterus.
