Orthognathic Surgery

Steps Involved in IVF:

Procedure Description

Orthognathic surgery is performed to correct various skeletal and dental deformities in the jaw and face. It is typically indicated for patients who suffer from conditions such as underbite, overbite, crossbite, open bite, or sleep apnea, which cannot be resolved through orthodontic treatment alone. The procedure is often combined with orthodontic treatment, such as braces, to ensure the teeth are properly aligned before and after surgery.

During the surgery, the surgeon cuts and repositions the jawbones, which may involve the upper jaw (maxilla), lower jaw (mandible), or chin (genioplasty). In some cases, all three areas may need to be adjusted. The bones are then secured with screws, plates, or wires to hold the jaw in the new position. This process not only corrects bite issues but also enhances the patient’s facial symmetry and appearance.

Orthognathic surgery can significantly improve a patient's ability to chew, speak, and breathe. For those with functional problems, this procedure is transformative, offering relief from chronic pain, speech difficulties, and issues related to sleep apnea. In addition to medical benefits, many patients undergo orthognathic surgery for aesthetic improvements, seeking a more balanced and harmonious facial structure.

Procedure Duration

Orthognathic surgery is a multi-phase process that typically spans several months to a few years, depending on the complexity of the case and the need for orthodontic treatment.

  • Pre-Surgery Preparation:

Before surgery, patients usually undergo orthodontic treatment for 12 to 18 months to align the teeth properly. This ensures that the teeth are in the correct position before the jaw is repositioned. The surgical team will use X-rays, 3D models, and photographs to plan the surgery meticulously. Detailed planning is crucial for achieving the desired functional and aesthetic outcomes.
  • Surgery Day:

The surgery itself typically takes three to six hours, depending on the extent of the corrections needed. It is performed under general anesthesia, and patients are usually required to stay in the hospital for one to three days after the procedure. During this time, they will be closely monitored for any complications, such as infection or difficulty breathing.
  • Recovery Phase:

The initial recovery period lasts about six weeks, during which time the bones heal in their new positions. Patients are advised to follow a soft-food diet and avoid strenuous activities. Full recovery, including the removal of orthodontic appliances and the complete resolution of swelling and discomfort, can take up to a year. Regular follow-up appointments with both the orthodontist and surgeon are essential to monitor progress.

Benefits

  • Improved Functionality: Orthognathic surgery corrects jaw alignment, enhancing chewing, speaking, and breathing.
  • Facial Aesthetics: The procedure improves facial symmetry, creating a more balanced and harmonious appearance.
  • Chronic Pain Relief: Patients with temporomandibular joint (TMJ) disorders or jaw-related headaches may experience significant pain relief.
  • Enhanced Quality of Life: Many patients report better self-esteem and confidence after surgery due to the functional and aesthetic improvements.
  • Treatment for Sleep Apnea: For individuals with obstructive sleep apnea, orthognathic surgery can expand the airway and reduce or eliminate the condition.

Risks & Considerations

While orthognathic surgery can provide life-changing benefits, it is important to be aware of the risks and considerations:

  • Infection: Like any surgery, orthognathic procedures carry the risk of infection. Proper post-operative care and hygiene can mitigate this risk.
  • Nerve Damage: There is a possibility of temporary or permanent nerve damage, particularly in the lower jaw, leading to numbness or tingling in the face or lips.
  • Swelling and Discomfort: Significant swelling and bruising are common after surgery, and it can take weeks to subside fully.
  • Dietary Restrictions: Patients are required to follow a strict soft-food diet for several weeks post-surgery, which can be challenging.
  • Long Recovery Time: The recovery process can be lengthy, requiring patience and adherence to follow-up care and orthodontic treatment.

How to Choose the Right Doctor and Hospital

Choosing the right doctor and hospital for orthognathic surgery is crucial to ensure a safe and successful outcome. Here are some practical tips for selecting the best medical team:

  • Check Credentials and Experience: Look for a surgeon who specializes in orthognathic surgery and has extensive experience in performing these procedures. Board certification and membership in professional associations are important indicators of a qualified surgeon.
  • Hospital Accreditation and Facilities: The hospital or clinic should have modern facilities equipped for advanced surgical procedures. Accreditation by reputable international health organizations is a sign of high-quality care standards. Additionally, ensure the hospital has an experienced multi-disciplinary team, including orthodontists and anesthesiologists, to provide comprehensive care before, during, and after surgery.

Frequently Asked Questions

What actually happens during hyperstimulation of the ovaries?

The patient will take injectable FSH (follicle stimulating hormone) for eight to eleven days, depending on how long the follicles take to mature. This hormone is produced naturally in a woman’s body causing one egg to develop per cycle. Taking the injectable FSH causes several follicles to develop at once, at approximately the same rate. The development is monitored with vaginal ultrasounds and following the patient’s levels of estradiol and progesterone. FSH brand names include Repronex, Follistim, Menopur, Gonal-F and Bravelle. The patient injects herself daily.

What happens during egg retrieval?

When the follicles have developed enough to be harvested, the patient attends an appointment  where she is anesthetized and prepared for the procedure. Next, the doctor uses an ultrasound probe to guide a needle through the vaginal wall and into the follicle of the ovary. The thin needle draws the follicle fluid, which is then examined by an embryologist to find the eggs. The whole process takes about 20 minutes.

What happens to the eggs?

In the next step, the harvested eggs are then fertilized. If the sperm from the potential father, or in some cases, anonymous donor, has normal functionality, the eggs and sperm are placed together in a dish with a nutrient fluid, then incubated overnight to fertilize normally. If the sperm functionality is suboptimal, an embryologist uses Intracytoplasmic Sperm Injection to inject a single sperm into a single egg with an extremely precise glass needle.  Once fertilization is complete, the embryos are assessed and prepared to be transferred to the patient’s uterus.

How are the embryos transferred back to the uterus?

The doctor and the patient will discuss the number of embryos to be transferred. The number of successfully fertilized eggs usually determines the number of eggs to be placed in the uterus. Embryos are transferred to the uterus with transabdominal ultrasound guidance. This process does not require anesthesia, but it can cause minor cervical or uterine discomfort. Following transfer, the patient is advised to take at least one days bed rest and two or three additional days of rest, then 10 to 12 days later, two pregnancy tests are scheduled to confirm success. Once two positive tests are completed, an obstetrical ultrasound is ordered to show the sac, fetal pole, yolk sac and fetal heart rate.

Embryoscope©

Built into this technology there is a microscope with a powerful camera that allows the uninterrupted monitoring of the embryo during its first hours of life. In this way, we can keep a close eye on the embryo, from the moment when the oocyte is inseminated and begins to divide into smaller and smaller cells, until it can be transferred to the uterus.
